(a) In addition to the requirements of section 86-29, each application for an escort bureau license shall contain the following information:

(1) If the applicant is an individual, the legal name and complete address of such individual and satisfactory proof that he or she is 21 years of age or older and has been a resident of the State of Colorado for six weeks immediately prior to the filing of the application;

(2) If the applicant is a legal entity, the legal name and complete address of such entity, the legal names and complete addresses of each of the individual officers, directors, managers, partners, members, and/or principal owners of such entity, the legal name and complete address of such entity's registered agent for service of process, if any, and satisfactory proof that each of the individual officers, directors, managers, partners, members, and/or principal owners of such entity are 21 years of age or older and have been a resident of the State of Colorado for six weeks immediately prior to the filing of the application;

(3) A complete disclosure of all persons having a direct or indirect financial interest in the applicant's business and the extent of such interest;

(4) Satisfactory proof of the applicant's ownership or right to possession of the premises wherein the escort bureau will be operated;

(5) Whether the applicant or any of the other individuals required to be listed in the application have been convicted of any criminal acts and, if so, the criminal act involved, the date and place of conviction, and the disposition;

(6) A copy of the criminal history record of the applicant and each of the other individuals required to be listed in the application obtained from the Colorado Bureau of Investigation;

(7) Whether the applicant or any of the other individuals required to be listed in the application holds any other licenses under this division or other similar escort bureau ordinances from another city or county, and, if so, the names and locations of such other licensed businesses;

(8) Whether the applicant or any of the other individuals required to be listed in the application has had a previous license under this or any other similar escort bureau ordinance from another city or county denied, suspended, or revoked, and, if so, the name and location of the escort bureau for which such license was denied, suspended, or revoked, as well as the date of such denial, suspension, or revocation; and

(9) Whether the applicant or any of the other individuals required to be listed in the application has been an officer, director, manager, partner, member, and/or principal owner of any legal entity which has had a previous license under this or any other escort bureau ordinance from another city or county denied, suspended, or revoked, and, if so, the name and location of the escort bureau for which such license was denied, suspended, or revoked, as well as the date of such denial, suspension, or revocation.

(b) Each escort shall submit an application for an escort license on a form to be provided by the licensing administrator. The application shall contain:

(1) The applicant's name, address, and telephone number;

(2) The name, address, and telephone number of the escort bureau by which the applicant is employed or with which the applicant is associated;

(3) Satisfactory proof that he or she is 21 years of age or older and is a resident of the State of Colorado;

(4) Whether the applicant has been convicted of any criminal acts and, if so, the criminal act involved, the date and place of conviction, and the disposition; and

(5) A copy of the criminal history record of the applicant obtained from the Colorado Bureau of Investigation.

(c) Each escort bureau runner shall submit an application for an escort bureau runner license on a form to be provided by the licensing administrator. The application shall contain:

(1) The applicant's name, address, and telephone number;

(2) The name, address, and telephone number of the escort bureau by which the applicant is employed or with which the applicant is associated;

(3) Satisfactory proof that he or she is 21 years of age or older and is a resident of the State of Colorado; and

(4) Whether the applicant has been convicted of any criminal acts and, if so, the criminal act involved, the date and place of conviction, and the disposition.

(5) A copy of the criminal history record of the applicant obtained from the Colorado Bureau of Investigation.

(d) Each application for escort bureau license, escort license, or escort bureau runner license shall be verified by the oath or affirmation of the applicant or the applicant's authorized agent. (Ord. No. 98-86, § 14, 11-30-1998)
