(a) The jury service certificate shall contain the following information:

(1) The name and address of the juror;

(2) The name and address of the municipal court;

(3) The number of days of jury service performed and the dates thereof;

(4) Any other information deemed appropriate by the jury commissioner.

Jury service certificates shall be given to each juror.

(b) Upon discharging or releasing a juror from service, the jury commissioner shall either present to or mail the juror a jury service certificate.

(c) Jurors shall receive payments due from the city for jury service by check. Each check shall include all compensation for jury service and shall be mailed to the juror as soon as practicable after the juror's service. (Code 1979, § 25-83; Ord. No. 2005-12, § 93, 4-11-2005)
