The application for every public right-of-way contractor's license required by this article shall contain the name of the person desiring such license; the residence of such applicant, if the applicant is a person, or the place of business of the firm or corporation, if applicable; and the experience and training, if any, that the applicant has had in the particular license area required by this article.
