(a) In addition to the requirements of section 86-29, each application for a pawnbroker license shall contain the following information:

(1) Whether the applicant or any of the other individuals required to be listed in the application have been convicted of any criminal acts and, if so, the criminal act involved, the date and place of conviction, and the disposition;

(2) Whether a judgment has been entered against the applicant or any of the other individuals required to be listed in the application in any civil court for activities arising out of the pawnbroker business and, if so, the type of claim involved, and the date, place, and amount of such judgment;

(3) Whether the applicant or any of the other individuals required to be listed in the application has had a previous license under this or any other similar pawnbroker ordinance from another city or county denied, suspended, or revoked, and, if so, the name and location of the pawnbroker business for which such license was denied, suspended, or revoked, as well as the date of such denial, suspension, or revocation;

(4) Whether the applicant or any of the other individuals required to be listed in the application has been an officer, director, manager, partner, member, and/or principal owner of any legal entity which has had a previous license under this or any other similar pawnbroker ordinance from another city denied, suspended, or revoked, and, if so, the name and location of the pawnbroker business for which such license was denied, suspended, or revoked, as well as the date of such denial, suspension, or revocation.

(5) Whether the applicant or any of the other individuals required to be listed in the application holds any other licenses under this division or other similar pawnbroker ordinances from another city or county, and, if so, the names and locations of such other licensed businesses; and

(6) Satisfactory proof of the applicant's ownership or right to possession of the premises wherein the pawnbroker business will be operated.

(b) Each manager of a pawnbroker business shall submit an application for a pawnbroker manager's license on a form to be provided by the licensing administrator. The application shall contain the applicant's name, address, date of birth, telephone number, and the information required by subsection (a)(1) of this section.

(c) Each application for a pawnbroker or manager's license shall be verified by the oath or affirmation of the applicant or the applicant's authorized agent.

(d) No application for a pawnbroker license shall be received or acted upon for any location which is within a two-mile radius of the location of any other pawnbroker business licensed hereunder; provided, however, that this restriction shall not apply to the renewal or transfer of ownership of any pawnbroker license issued on or before July 18, 1994, for the location identified in said license that is in compliance with section 146-1246(B) of this Code relating to zoning.
