(a) In addition to the requirements of section 86-29 each application for an after-hours club or teen club license shall contain the following information:

(1) If the applicant is an individual, satisfactory proof that he or she is 21 years of age or older;

(2) If the applicant is a legal entity, satisfactory proof that each of the individual officers, directors, managers, partners, members, and/or principal owners of such entity are 21 years of age or older;

(3) Whether the applicant or any of the other individuals required to be listed in the application have been convicted of any criminal acts specified in section 86-370(a)(2) and, if so, the criminal act involved, the date and place of conviction, and the disposition;

(4) Whether the applicant or any of the other individuals required to be listed in the application has had a previous license under this or any other similar after-hours club or teen club ordinance from another city or county denied, suspended, or revoked, and, if so, the name and location of the after-hours club or teen club for which such license was denied, suspended, or revoked, as well as the date of such denial, suspension, or revocation;

(5) Whether the applicant or any of the other individuals required to be listed in the application has been an officer, director, manager, partner, member, and/or principal owner of any legal entity which has had a previous license under this or any other similar after-hours club or teen club ordinance from another city or county denied, suspended, or revoked and, if so, the name and location of the after-hours club or teen club for which such license was denied, suspended, or revoked, as well as the date of such denial, suspension, or revocation;

(6) Whether the applicant or any of the other individuals required to be listed in the application holds any other licenses under this division or other similar after-hours club or teen club ordinances from another city or county, and, if so, the names and locations of such other licensed businesses;

(7) Satisfactory proof of the applicant's ownership or right to possession of the premises wherein the after-hours club or teen club will be operated;

(8) If the building wherein the after-hours club or teen club will be operated is in existence, complete plans and specifications for the interior of the building; and

(9) If the building wherein the after-hours club or teen club will be operated is not in existence, complete plans and specifications for the interior of the building and an architect's drawing of the building to be constructed.

(b) Each manager of an after-hours club or a teen club shall submit an application for a manager's license on a form to be provided by the licensing administrator. The application shall contain the applicant's name, address, date of birth, telephone number, and the information required by subsection (a)(3) of this section.

(c) Each application for an after-hours club, teen club, or manager's license shall be verified by the oath or affirmation of the applicant or the applicant's authorized agent. (Ord. No. 98-86, §§ 12, 13, 11-30-1998)
