(a) A jury questionnaire shall be given to each juror on or before the date the juror's service is to commence. The information requested shall be information used by the court or jury commissioner to assist in the completion of its duties and shall include the following:

(1) Name, sex, age, residence, and marital status;

(2) The number of children;

(3) Occupation; whether the juror is regularly employed;

(4) Spouse's occupation;

(5) Previous juror service;

(6) Present or past involvement as a party or witness in civil or criminal proceedings; and

(7) Such other information as the court or jury commissioner deem appropriate.

The questionnaire shall contain a declaration by the juror that the information supplied is to the best of the juror's knowledge true and acknowledged and that a willful misrepresentation of a material fact may be punishable in accordance with the contempt power of the court. Immediately below the declaration, the questionnaire shall contain a place for the signature of the juror. A notice that the completed questionnaire is not public record shall appear prominently on its face.

(b) The jury commissioner shall provide information from completed questionnaires deemed appropriate by the trial judge to counsel for use during jury selection. With the exception of the names of qualified jurors and disclosures made during jury selection, this information shall be held in confidence by the court, the parties, trial counsel, and their agents. Upon the completion of jury selection, this information shall be returned to the court. The original completed questionnaires for all prospective jurors shall be retained in the court's file for 60 days after the completion of the juror's service. If at the expiration of the 60-day period no appeal is pending which involves an issue of juror qualification or a challenge to the panel or the pool, the questionnaires shall be destroyed. Juror questionnaires shall not be considered public records as defined in C.R.S. tit. 24, art. 72(C.R.S. § 24-72-101 et seq.).

(c) Upon request, the jury commissioner shall make available for inspection by parties, counsel, their agents, or members of the public a list of prospective jurors containing the juror's names and addresses, unless otherwise ordered by the court.
