The application for every tree trimmer's license required by this division shall contain the name of the person desiring such license; the residence of such applicant, if the applicant is an individual, or the place of business of the firm or corporation, if applicable; the trimming, pruning or removing of trees or shrubs; and the number of persons employed by the applicant.
